FOOD PERMITS and APPLICATIONS
Food Permits are required for any public event serving foods for any purpose. Below are the Food Permit forms required for that process. If you are considering holding an event where the public will be in attendance and may offer food (for free or a fee, tastings, etc.), please contact the Board of Health office for further assistance, if needed.
